To Missions Donors: The following information has been provided to assist you in determining whether gifts you make in connection with a mission trip sponsored by Calvary Chapel, are deductible for federal income tax purposes. You should consult your personal tax advisor regarding the deductibility of any gifts you make related to Calvary Chapel's Youth Mission Trips.

The IRS regulations state that gifts made to individuals are not tax deductible. Therefore, please do not make checks payable to a specific team member or staff person when contributing funds for Calvary Chapel Mission Trips. A tax deduction may be denied if an individual's name appears on the check.

You may make a gift of funds directly to a team member if you do not intend to deduct the gift as a charitable contribution on your personal income tax return.

Please do not send postdated checks.

Receipts will be mailed at year-end. Calvary Chapel does not issue contribution acknowledgment receipts for cash donations. If you wish to receive a contribution acknowledgement for your gift, please either send a check or purchase a money order or cashier's check, made payable to Calvary Chapel.

All charitable contributions received in connection with mission trips are under the full discretion and control of Calvary Chapel and are NON REFUNDABLE. In the event that a team member raises contributions in excess of the mission trip expenses or decides not to go and cancels, Calvary Chapel will use the money towards another team member or supplies needed for missions.

3. Fill in Donation Amount.
4. Choose mode of payment, using Credit Card, Debit Card or Checking Account. Sorry, we do not accept American Express.
5. Keep your receipt for this donation.
